The Associate Care Manager (ACM) is responsible for providing care management services to Seabury Care Management clients under the guidance of senior Care Managers and supervisors in order to grow in their proficiency as a Care Manager. This position requires significant time working with care management clients through home visits, and requires detailed timely documentation with regard to client records, care plans and service notes. This position also includes regular educational sessions designed to build the ACMs skills into a Care Manager. The ACM reports to a care management supervisor. The Care Manager works from a home office and will need to use a computer, enter client service notes into Seabury database, and provide her /his own internet service, cell phone and transportation/car.

- Provides Care Management services to clients as assigned and agreed upon.
- Completes client assessments, care plans and provides on-going monitoring and coordination of services following Seabury Care Management protocols and procedures and the instruction of Seabury Care Management Supervisors.
- Attends monthly team meetings, case review and meetings with supervisor as scheduled.
- Meets expected care management client service units based on agreed upon commitment of work time. Full time or Part time (50% or more) position available.
- Administrative tasks expected with those service units including:
- Inputs service notes into case note system for each client within 48 hours of contact with, or completion of work on behalf of client.
- Edits service notes for each client served by the 3rd day of the following month.
- Maintain personal client files and keep organized.
- Maintains administrative notes regarding education and resource trainings.
- Informs the Care Management team about new services and resources in the community via email and during attendance at team meetings.
- Provides weekend/holiday coverage of phones and emergencies on a rotating basis with other administrative staff and care managers. This would begin later during the first year of employment/training and build to 6 times per year.
- Participates in intensive training modules to increase knowledge about care management practice. This also benefits the individual with obtaining advanced licensure and advanced Aging Life Care Association membership.
- Opportunity to accept assignment of additional work such as:
- Vacation coverage or other coverage situations for other Care Managers.
- Outreach and networking to inform the community of Care Management services.
Minimum Qualifications:
- Master’s degree-prepared social worker currently working toward licensure in the District of Columbia and/or Maryland, with ability to maintain licenses (clinical license preferred), or; Master’s degree in related field with accreditation from, and membership in the Association of Aging Life Care Professionals.
- Working knowledge of Microsoft Office applications; ability to learn case management software, database, etc.
- Excellent oral, written and interpersonal communication skills; effective email and Internet skills.
- Possessing strong organizational skills, good record keeping habits, and ability to set priorities and meet goals.
- Some knowledge and experience with issues of self-determination, strengths perspective and trends in social work.
- Some knowledge of or interest in older adults and aging issues.
- Have means and ability to travel primarily within the District of Columbia, Montgomery and Prince George’s Counties, Maryland and Northern Virginia.
- Good problem solver who is able to analyze problems and make recommendations to solve them.
- Maintain liability insurance, valid driver’s license and car insurance.
- Self-starter and leader.
Work Environment: Home office, frequent local travel to client’s homes.
Essential Physical Functions: Ability to lift 20 pounds; ability to drive a vehicle (along with possession of a driver’s license) and ability to sit for long periods of time.
Equipment Used: Contractors are expected to have their own PC/laptop, internet, Smartphone with the ability to talk, text and check email while in field.

Compensation: $68,000.00 per year
